BOTOX®BOTOX® Cosmetic treatments can smooth dynamic wrinkles in the neck and face for a more youthful and rested appearance. Dynamic wrinkles are those caused by facial expressions and movements. Wrinkles that occur when the face is still cannot be removed by Botox. However, Botox can prevent further progression of wrinkles caused by frowning or smiling. A face that appears to express permanent anger, sadness or worry can be changed to one of contentment and calm. Botox is frequently used on the thin wrinkles that surround the eyes (often called “crows' feet”) and the mouth, chin and jaw. The aging face can be rejuvenated by the use of Botox, but younger faces can benefit as well because Botox will prevent the formation of dynamic wrinkles in men and women in their 20s and 30s. Some examples of regular people who benefited from Botox: Botox Case 1: JS is a nurse in her mid forties and feeling her age. She has some lines on her forehead which seem to be deepening, and two straight vertical lines above her nose which are provoked by frowning and squinting (she doesn't always like to use her glasses). Botox was applied to her forehead to prevent deepening of the lines already there. Botox was also applied to the area above her nose. When the Botox began to take effect 3 days later, AS said it felt “a little strange” because she could not contract those muscles when she frowned. She got used to it by the end of the day and was very happy with the results of her Botox treatment.
Botox Case 2: 30 year old HJ hated to smile because he thought the crinkling effect at the corners of his eyes made him look old. He is in sales, a field where friendly smiles are important. “A little crinkling is okay—I want to look normal and have some expression, but I really hate it when I smile and see a million lines there,” he said on his first visit. A small amount of Botox was applied to the outer corners of his eyes. Now he has some natural creasing when he smiles but the crow's feet are gone. “My friends can't believe I had Botox because I don't look frozen. I love it!” |

Botox Case 3: DG is in her early twenties and works in publishing. She has no permanent wrinkles on her skin and she wants to prevent them from forming. She has had Botox treatments regularly in the past year and no-one knows it. “My friends are spending tons on fancy skin creams and lotions and even though they're young, I see some wrinkles forming already. I get Botox because it works and it ends up being more cost-effective.”
Botox Case 4: FA is an administrative clerk in his mid 30s. He has had problems all his life with overactive sweat glands. “It's been embarrassing,” he said. “Sorry to be gross, but I go out on a date and we're having dinner in an air-conditioned restaurant and my armpits are dripping.” He was treated with Botox in both armpits and now, “I feel normal,” he says. “It's changed my life.”
What to expect: At your initial visit, a medical history and evaluation will be undertaken to determine if Botox is right for you. If you are a candidate for Botox, the doctor will discuss where it will be applied. After the face is cleansed, Botox will be applied to the appropriate areas on your face with a very fine needle that most patients do not even feel. After your Botox treatment you will be reminded not to rub the sites that were treated. Other than that, you may resume your normal activities. The effects of Botox can be seen as early as one day after treatment, though most people see results in 3-5 days. In some, it may take up to 2 weeks for the effects of Botox to be noticed. Risks/Side effects You should not have Botox treatments if you are pregnant, breastfeeding, have a neuromuscular disease such as myasthenia gravis, or are allergic to Botox. As with any other procedure in which the skin is punctured with a needle, Botox injections can cause buising, swelling and infection, though these complications are rare. How long does it last? Botox treatments are not permanent and last anywhere from 3-4 months. In some individuals, Botox can last up to 6 months. If Botox is used regularly as part of your skin care regimen, the amount of Botox used tends to decrease over time and may last longer with regular use.
Cosmetic dermal fillers are used to contour and sculpt the face by adding volume to smooth out wrinkles, fill in hollow areas under the eyes or in the cheeks and to add fullness to the lips. Fat loss, thinning of the face and sagging skin are normal consequences of aging which can be temporarily reversed by the use of injectable dermal fillers. Hyaluronic acid is a natural component of skin and acts like a sponge to attract and hold moisture in healthy skin. Aging results in decreased amounts of hyaluronic acid in the skin, resulting in coarsened skin, fine lines and wrinkles. Hyaluronic acid-based fillers are well-tolerated and can last 8-12 months, depending on the area of face filled and the individual. Allergies are extremely rare; a skin test is not required. Examples of this type of filler include Restylane and Juvederm. Radiesse, another temporary dermal filler, is an exciting alternative to hyaluronic acid which can produce dramatic results in filling in the nasolabial folds (“parentheses” lines beside the nose to the lips), cheeks and other areas of the face. Radiesse lasts 1 year or longer and is very effective in filling in deep lines and wrinkles of the face. What to expect After a medical history and evaluation is obtained, your doctor will discuss the best filler for your face. The face is cleaned and the appropriate dermal filler will be injected into your face. The procedure is well-tolerated because a very fine needle is used. If you prefer, you can also be injected with a local anesthetic to numb up the face while the doctor is working on you. If you have a history of oral herpes infection (“cold sores”), you will be given a prescription for medication to prevent an outbreak after the procedure. You will be reminded not to rub the treated areas of your face after your procedure. You can use make-up the next day and resume normal activities. Risks/Side Effects There will be bruising and swelling after the procedure that can last 3-4 days, and up to 7 days after your procedure. As with any other injection procedure, there is a risk of infection, though this is very rare.
Case Study: DD DD is in her fifties and had recently lost a lot of weight, which was good for her health but made her look gaunt in the face. She received Radiesse to the nasolabial folds (the lines on each side of the face that run from the nose to the corners of the mouth) and in her cheeks. The results were immediate and rewarding and she was thrilled. She can expect the results of her Radiesse filler to last a year or more.
Chemical Peels involve the application of special chemical formulations on the face for smoothing the texture of the skin by removing the outer, damaged layers. Acne, wrinkles and uneven pigmentation can all benefit from chemical peels. In addition, chemical peels can remove non-melanomatous pre-cancerous skin conditions, soften facial scars, reduce or eliminate age spots and freckles, minimize pore size and stimulate collagen production for younger, firmer skin. It is our belief that everyone should experience the benefits and preventative care of a chemical peel. Earth and Sage uses the innovative ViPeel, specially developed to address a wide range of skin types, including ethnic skin pigmentation that has usually had limited results with regular peels. Unlike other peels of similar strength, the ViPeel is well-tolerated and requires little ‘down time' so that normal work and social activities can be resumed immediately after treatment. The peeing process itself takes place over the course of a week and is very well-tolerated by our patients. What to expect After a medical history and evaluation are obtained, your doctor will clean the face to prepare it for the ViPeel. When the ViPeel is applied, you may experience some burning and stinging, but this goes away after a few minutes as the skin becomes anesthetized. The face may take on a tan or reddish coloration after the ViPeel is applied. You should leave the ViPeel on for 4-6 hours and follow the simple after-care instructions provided. Risks/Side Effects You might experience prolonged burning and stinging from the peel. Often this is due to the dryness of the skin induced by the peel and the ViDerm moisturizer included in your peel kit will help relieve that sensation. There is a rare risk of infection. In some skin types, there might be post-peel hyperpigmentation, which can be treated by prescription strength skin care products. Your skin will be more sensitive to sunlight after the peel and you should be vigilant about your use of sun-screen during the daylight hours. Pulling off the flaking skin can cause scarring, so you should leave the skin to peel on its own in its own time.
LIPOLYSIS MESOTHERAPY (CHEMICAL BODY-SCULPTING) In this simple, non-surgical office procedure, fat can be eliminated by the process of injections into the skin to destroy fat cells. While nutrition and exercise are important in maintaining proper weight, fat-dissolving mesotherapy can help remove stubborn fat on the abdomen, inner thighs and arms that are not corrected by diet and exercise. Mesotherapy has also been effectively used to dissolve lipomas (benign fatty tumors), and to correct unsatisfactory results from previous liposuction procedures. Results can be seen in 4 to 8 sessions, depending on the individual and the amount of fat to be removed. What to expect A thorough medical history and evaluation will be taken and if you are determined to be a candidate for the procedure, a mixture of fat-dissolving chemicals and supplements will be injected into the fatty areas. Redness and swelling of the injected areas are normal and expected reactions to the procedure. There may be some temporary itching and soreness (some patients have described this as an “achiness” ) at the injected sites. Most people resume normal activities the next day, though some may lay off heavy exercise for a day or two until the soreness disappears. Risks/Side Effects As with any injection procedure, there is the rare risk of infection. Redness and swelling at the injection sites are normal and expected.
Medical Skin Needling is used to stimulate the skin to produce new collagen and elastin for a younger, more vibrant appearance. Skin Needling can also help smooth out depressed acne scars and improve the appearance of stretch marks and hypopigmented scars. There is minimal downtime to Skin Needling and it is very safe. Because the epidermis is not ablated but remains intact, Skin Needling does not cause burns to the skin like ablative lasers. The effects can last years. Best results are seen with 4-8 treatments, though many have seen smoothening with only 2 Skin Needling sessions. Areas to be treated include the face, neck, hands, décolleté. What to expect As this is a medical procedure and a medical history and evaluation will be taken from you before you undergo Skin Needling. The area to be needled is cleaned and if needed, numbing cream is applied for 15 minutes. A very fine needle is used to penetrate the skin to stimulate collagen formation. Often, vitamins and minerals are injected directly onto the skin during the Skin Needling treatment so that these supplements can be delivered directly into the skin for maximal antioxidant effect. Risks/Side Effects There are minimal risks of bruising, swelling and infection. However, most patients report almost zero downtime and many request a Skin Needling session the day before an event to capitalize on the “glow effect” that accompanies Skin Needling.
SCLEROTHERAPY (TREATMENT OF LEG VEINS) If you've been embarrassed by “spider veins” or “maps” on your legs, sclerotherapy can help. In this procedure, a solution is injected into the unsightly veins to cause localized inflammation and “shut them down”. This is safe because these veins are formed as a result of collateral circulation resulting from a combination of genetic and lifestyle factors. The main veins that are important for maintaining proper circulation to the leg are not affected. Optimal management requires anywhere from 2-6 sessions on average. What to expect After a medical history and evaluation have been obtained, the area to be injected is cleaned. A very fine needle is used to inject the “spider veins”. There is minimal pain (some patients have fallen asleep during treatments) though some may experience an occasional burning sensation. After the procedure you should walk around for 20-30 minutes. You should not bathe or shower with hot water (warm or tepid water is fine) and should splash cold water on your legs at the end of your shower. You should use sunscreen at all times as sunlight can provoke the formation of new “spider veins”. Risks/Side Effects As with any injection procedure, there is the rare risk of infection. There might be minimal swelling and bruising. Some people may experience hyperpigmentation (brown spots) at the injection sites. Most of the spots disappear on their own within a year.
